Make a difference each and every day in the role of Case Manager in Aged Care.

* Deliver person-centred case management to clients that enables independence and promotes improved outcomes.
* Manage a caseload of clients under CHSP/HCP approved funding, maintaining budgets and onboarding growth of new clients.


About You:

* Experience in delivering quality, client-focused case management with an understanding of various funding sources available in the Aged Care sector.
* Tertiary qualifications in Community Services, Case Management or related disciplines with high level computer systems proficiency.
* Develop care plans for complex clients and hold current First Aid qualifications (or willingness to obtain before commencement).
* A reliable, comprehensively insured vehicle and Australian driver's license are also essential.
