About the role
Lead the planning, coordination and monitoring of high-quality, individualised, client-centred care and services.
This position is responsible for leading case management and the delivery of person-centred home and community care services. In this role, you will facilitate the delivery of person-centred quality services to support our clients to live in their homes for longer and stay connected to their local community. The role will be the central point of contact for clients and be responsible for care planning services to support individual needs based on available funding.

Selection Criteria

* Demonstrated experience (minimum 3 years) in case management within the aged/home care sector, including co-designing care plans and conducting assessments.
* In-depth knowledge of the Aged Care Quality Standards, the Support at Home Program, and relevant compliance requirements.
* Proven ability to manage client budgets and resources effectively to achieve person-centered goals.
* Strong leadership and mentoring skills, with experience in supervising, training, and supporting care staff.
* Excellent interpersonal, communication, and conflict-resolution skills, including experience in liaising with diverse stakeholders and managing risk.
* Proficiency in using electronic client management systems.
