Job Overview
The Senior Case Manager plays a vital role in assessing and managing complex cases, ensuring effective interventions are implemented to address offending behavior and facilitating community referrals when necessary. This position requires coordination with the community support sector, maintaining strong linkages and providing critical information to the courts to inform bail and sentencing decisions.
Key Responsibilities
* Provide expert advice on programs and supports available through Specialist Courts and Programs.
* Manage a caseload of participants with intricate needs, conducting thorough risk assessments and developing comprehensive case management plans under the guidance of the Team Leader.
* Cultivate and maintain relationships with key stakeholders, including government and non-government organisations, to facilitate connections and access to essential services.
* Apply sound judgment and best practice principles to manage a caseload effectively, ensuring participant activities align with evidence-based methodology, under the oversight of the Team Leader.
* Offer detailed verbal and written advice, including progress reports and final documentation, to assist with bail and sentencing decisions.
* Engage with the judiciary, legal profession, and court staff to manage participants within the program and provide informed advice on available services and programs.
