Job Description:

As a Case Manager, you will be responsible for delivering person-centred and enablement-focused case management to clients. This role involves care coordination, administrative tasks, and rostering of client services.



Key Responsibilities:

* Manage a caseload under approved funding
* Maintain budgets
* Onboard new clients
* Ensure ongoing care plans support client independence



Requirements:

Experience in delivering quality, client-focused case management is essential. A strong understanding of various funding sources available in the aged care sector is also required.



Qualifications:

A tertiary qualification in Community Services, Case Management or related discipline is necessary for this role. Additionally, a high level of computer systems proficiency, including data entry, is required.
