Liquid Personnel is currently recruiting for experienced Social Workers to join the Adult's Hospital Discharge Team at Liverpool City.
Responsibilities
You will support individuals during the hospital discharge process, ensuring their transition from hospital to the community is safe, efficient, and person-centered. You will carry out assessments, develop care plans, and work collaboratively with multidisciplinary teams to provide tailored support to meet each individual's needs.

Qualifications and Experience
To succeed in this role, you must have Social Work England registration and be eligible to work in the UK. You should hold a qualification in Social Work at degree level (or equivalent) and have post-qualified experience. A valid UK driving license is also required.
