**About the service**

Barwon Health's Adult Continuing Care Services (CCS) provides community supports to people aged between 16 and 65 years, experiencing serious mental health needs.

Our service delivery includes comprehensive crisis assessment, crisis management and treatment, case management of complex issues and mobile outreach. We have community case teams situated in five locations across the Barwon region ranging from Corio and Geelong West to Bellarine, Torquay & Colac. Each team is situated in a community health centre that is close to the city centres, beaches or local community hubs.

**About you**
- A demonstrated high level of clinical skills in the assessment, treatment and continuing care of adults with mental health disorders in a community setting
- Thorough knowledge of the objectives and provisions of the Mental Health Act (2014) and amendments
- Current Victorian Driver Licence

**About the roles**

As the successful applicant your responsibilities include facilitating assessment and service provision and providing coordination of care, whether hospital or community based, or across different types of services. The clinician's role includes;
- Development of a comprehensive recovery and treatment plan with the consumer
- Outreach activities
- Consumer and carer education, support and follow-up
- Assessment of consumers' medical, psychosocial and current service needs
- Development, implementation and evaluation of Treatment & Care Plans
- Advocacy
- Referrals and assistance with linkage to medical, NDIA, mental health, substance use services and psychosocial services as needed
- a) _Clinician - Torquay - 56 hours per fortnight, temporary (parental leave relief)_
- b) _Clinician - Newcomb - 48 hours per fortnight, temporary until 15/9/23_
- c) _Clinician - Newcomb - 80 hours per fortnight (+ADO), temporary until 29/2/24_
- d) Clinician - Torquay - 80 hours per fortnight (+ADO), temporary until 14/7/24_
- e) Senior Clinician - Newcomb - 40 hours per fortnight (parental leave relief)_
- Due to the number of vacancies, applicants will be interviewed on an ongoing basis and offerred positions accordingly. We will endeavour to update the above available positions regularly._

Essential: Formal professionally recognised qualifications in Psychiatric Nursing, Occupational Therapy, Social Work or Clinical Psychology
Essential: Relevant professional registration with discipline specific agency
Essential: Ability to work a rotating 7 day a week roster including; 0800-1700 M - F, 1130-2000 evening weekdays and 1030-1900 on public holidays and weekends.
