Currently, there are around 5.5 million Australians with a disability. The National Disability Insurance Agency (NDIA) implements the National Disability Insurance Scheme (NDIS), which is one of the biggest social reforms in the country since Medicare. As an Agency, we support more than 717,000 participants with a significant and permanent disability, and the families and carers of those living with disability. For many Australians, this will be the first time they receive the disability support they need. The NDIA is designed to enhance the quality of life and increase economic and social participation for people with disability and will mean a peace of mind for every Australian.

The Complex Support Needs Branch provides specialised support for people with disability with additional complexity due to situational and personal factors.

You will be considered for all roles, however in the application form we will ask you to select a role and preferred location to understand your preferences.

Aged Care and Hospital Interface Branch

* APS6 Planners – Aged Care and Hospital Interface
* APS6 Hospital Liaison Officers - Hospital Liaison Team

The Aged Care and Hospital Interface Branch is responsible for servicing several specialized cohorts of participants. This includes participants living in or at risk of entering aged care, those diagnosed with MND, participants with terminal illness and disability, and supporting those in hospital to discharge.

You will be considered for all roles, however in the application form we will ask you to select a role and preferred location to understand your preferences.

APS6 Planner - Complex Support Needs:
The Complex Support Needs Planning Teams provides planning support for participants with complex support needs.

APS6 Planner - Justice Team:
The Justice Planning Team provides support to participants who intersect with the justice system, with the aim of ensuring participants have the disability supports they require whilst in custody, and to enable informed transition to the community.

APS6 Justice Liaison Officer - Justice Liaison Team:
The Justice Liaison Team works nationally to provide timely and accurate support to internal and external stakeholders working with people with disability interacting with the justice system within a correctional facility.

APS6 Hospital Liaison Officer - Hospital Liaison Team:
The Hospital Liaison Team works nationally to provide timely and accurate support to internal and external stakeholders working with people with disability interacting with the health system.

APS6 Planner – Aged Care & Hospital Interface Branch:

Planners in the Aged Care & Hospital Interface branch, work directly with participants to provide high quality planning support either via case management or to support those in hospital to discharged.

* Apply strong cultural competence, showing respect for cultural diversity
* Adapt with confidence, responding flexibly to changing environments
* Uphold professionalism and integrity, maintaining high ethical standards, sound judgement
* Exhibit excellent time management, effectively coordinating weekly participant meetings and responding proactively to any risks or escalations that arise

While these roles are ideally performed from Townsville, Ipswich, Port Augusta, Port Lincoln, Mt Gambier, Port Maquarie, Surry Hills, Newcastle, Hobart, Launceston and Deakin, remote working arrangements may be considered. We intend to fill this role in an ongoing capacity, however non-ongoing roles up to 12 months with the possibility of extension up to 2 years may be offered.

Successful candidates will be required to undergo a pre-employment National Police History check, Worker Screening check and/or Working with Children Check (where required) and provide evidence of Australian Citizenship.
