Armajun Aboriginal Health Service in is currently looking for a full-time, motivated, Aboriginal Health Worker, to join their team in Inverell.
Armajun Aboriginal Health Service works closely with communities to provide a quality and culturally appropriate health service with support in General Practice, Chronic Disease management and other Allied Health services.
There is currently an opportunity for an Aboriginal Health Worker (AHW), to become part of this team of passionate and dedicated health professionals. The AHW will provide direct support to Aboriginal community members and their families, improving and facilitating their access to services, including primary health care, culturally appropriate educational and promotional programs, as well as enabling best practice care to be delivered by relevant service providers. The position will also assist to identify health and social determinants contributing to clients developing chronic health conditions, supporting referral pathways and support strategies developed to improve health and life outcomes. The position also includes providing transport for clients to attend appointments. This is both a clinical and non-clinical role.
