The successful applicant will hold responsibility for the care and management of a defined clinical caseload of patients within a very busy District Nursing Team.
There are multiple demands in this exciting role due to the challenging nature of providing care for patients outside of the hospital environment.Main duties of the jobThe post holder will be responsible for providing support to the Team Leader in the day-to-day management of multidisciplinary / multi-agency packages of scheduled and unscheduled care for patients with multiple, complex, and palliative care needs.
