About The Royal Melbourne Hospital
The Royal Melbourne Hospital (RMH) is part of the Parkville Precinct and is one of Australia\'s leading public hospitals, providing a comprehensive range of specialist medical, surgical, and mental health services, as well as rehabilitation, aged care, specialist clinics and community programs. RMH Cardiology are seeking an experienced Cardiology Liaison Nurse to join our team. The Cardiology Department includes the Coronary Care and Step-down Unit, the Cardiac Catheter Laboratories, a Cardiology Ward, a Cardiology day ward, and the non-invasive and diagnostic service. The Department provides care to emergency and elective patients with heart disease including ischaemic heart disease, heart failure and valvular heart disease.

About The Role
* Management/triaging of Cardiology elective referrals, inter-hospital transfers and Cardiology outpatient referrals.
* Coordinate and organise any necessary pre-operative investigations. Liaising with the medical team and GPs to ensure all appropriate investigations are carried out for preadmission and country referral patients.
* Liaise with multidiscliplinary team to facilitate timely elective admissions and transfers of care and discharges.
* Coordinate the booking process, including patient pre-procedure assessment, plan of care and identifies problems.
* Coordinate, negotiate, procure and manage the transition of care for selected patients admitted from other health services.
* Develop strategies to improve waiting lists and streamline the elective booking process, including pre-admission booking and assessment, care planning and discharge planning, risks and co-morbidities.
About You
* Nursing registration with AHPRA – registered nurse
* Post graduate certificate/Diploma in acute cardiac care
* Recent acute cardiac clinical care of at least 5 years
* Excellent communication, negotiation and interpersonal skills at all levels
