Comprehensive Support Specialist
* Deliver tailored support and intensive case management to individuals experiencing homelessness, with a focus on long-term housing stability and independent living.
* Conduct holistic assessments to develop goal-oriented plans, incorporating strength-based approaches to promote resilience and self-sufficiency.
* Provide expert care coordination and service navigation, connecting clients with mainstream and specialist services, including health, mental health, and AOD support.
* Offer targeted support to clients with increased vulnerabilities and complex needs, adopting a flexible and responsive approach to address emerging challenges.
* Collaborate with other agencies, participate in community networking, and convene multi-disciplinary team meetings to ensure integrated client support.
Key Requirements
* Proven experience in case management and support work, with a strong understanding of homelessness issues and effective interventions.
* Excellent communication and interpersonal skills, with the ability to build rapport with diverse clients and stakeholders.
* Strong assessment and planning skills, with the capacity to develop and implement individualized support plans.
