Located on the beautiful NSW North Coast approx. 6 hours' drive from Sydney and 3.5 hours from Brisbane our region offers rainforests, epic headlands, sand dunes and some of the best Australian beaches with a variety of multi-cultural cafes, restaurants, and friendly community towns. Make an impact at Serco The health services at Clarence have 3 modern health Centres that deliver appropriate health care standards to approximately 1700 inmates from diverse backgrounds and cultures. You will provide primary care based on a patient-centred model allowing patients to participate in their health management actively. The team works closely with correctional staff to deliver holistic and integrated patient-centered health services that: Promote the rehabilitation and reintegration of inmates. Ensure the safety and security of inmates. Fosters a high standard of practice to facilitate safe healthcare delivery. Serco is a place for you The Registered Nurse – Chronic Disease is responsible for identifying, managing, and supporting patients with chronic health conditions in a custodial environment. This includes conducting assessments, developing care plans, coordinating referrals, and delivering health education to improve long-term outcomes and quality of life. The role works closely with a multidisciplinary team to ensure culturally sensitive and evidence-based care. To be successful in this role, you’ll have: Current registration with AHPRA as a Registered Nurse Minimum 2 years' post-graduate experience, ideally in chronic disease, primary health, or correctional health Post-graduate qualifications in chronic disease (or working towards) desirable Please note: This position carries trust and responsibility.
