Job Title: Registered Nurse
We are seeking a skilled and compassionate Registered Nurse to join our Community Nursing team at Wyong Central Community Health Centre. As a key member of our team, you will play a vital role in providing high-quality patient care and making a positive impact on the lives of our community.
Key Responsibilities:
* Provide home visits throughout the community, working collaboratively with clinicians to facilitate case management and care planning.
* Develop and maintain responsive strategies to meet the changing health needs of the community, recognising the importance of social and ecological influences on individual health.
* Work autonomously and collaboratively as part of a multi-disciplinary team, delivering high-quality, evidence-based care in wound management, catheter and drain care, and palliative support.
Requirements:
* Current registration as a Registered Nurse (Division 1) with AHPRA and hold a current Australian driver's license.
* Recent nursing experience in Community Nursing or demonstrated interest in Community Nursing.
* Demonstrated ability to liaise with other health professionals to facilitate case management and care planning.
* Strong focus on developing key relationships in a multi-disciplinary team and enjoyment of working collaboratively.
