Overview
Are you passionate about making a difference in people's lives? We are seeking dedicated individuals to join our team as Community Support Workers working on a casual basis in Streaky Bay. Eyre and Far North Local Health Network has moved to a Consumer‐Directed Care model for the provision of aged care, disability, and support services. Consumers maintain greater control over the services they receive, participate in setting goals, and manage their own care arrangements. The Community Support Worker supports this model by providing personal care, medication support, cleaning, meal preparation, transport and social support to clients who are aged, frail or disabled and wish to remain living in their own home with assistance, or are living in a Residential Aged Care facility. At the WHA‐4 level, you will provide personal care and support to clients with complex care needs. Community Support Workers need to be flexible, conscientious and empathetic, and be able to work autonomously in the community. They will work closely and collaboratively with the consumer, their family and other health professionals to provide quality outcomes and maintain consumer independence where possible. To be appointed to the WHA‐4 level you must have completed Certificate III or higher in Individual Support (Aged Care, Disability or Home and Community).
