Clinical Hospital Pharmacist Role
A clinical hospital pharmacist is required to provide high-quality medication management services within a supportive and collaborative team environment.
* Prepare comprehensive Best Possible Medication History lists for hospital inpatients.
* Conduct thorough medication chart reviews and medication reconciliation to ensure patient safety.
* Dispense and supply medications and pharmacy products efficiently.
* Coordinate patient medication requirements for inpatient stays, day infusions, and discharges effectively.
* Develop and maintain professional relationships with key stakeholders to deliver excellent customer service.
* Support patients, carers, hospital staff, and associated employees.
* Educate and advise staff and customers on health promotion, disease prevention, drug selection, and proper use of medicines.
Requirements:
* Hold full general registration as a pharmacist in Australia (AHPRA) or New Zealand (PCNZ).
* Prior experience in dispensing/clinical roles within Australian or New Zealand-based hospitals or community pharmacies is essential.
* Experience in a clinical setting is highly regarded.
* Strong knowledge of the Pharmaceutical Benefits Scheme (PBS) is also highly valued.
* Effective time management, communication, and teamwork skills are necessary.
* Flexibility, adaptability, and commitment to ongoing learning are expected.
* Attention to detail and ability to work autonomously are critical.
