Footprints Community is a well-regarded not-for-profit provider of community-based services, working in the community for over 30 years. We specialise in working with older people, those that experience disability, mental illness, as well as those who are at risk of homelessness.
About the role and program
This position is responsible
for delivering the WTTCC program through trauma-informed, recovery-oriented,
strengths-based, and person-centred case management. The role involves working
collaboratively with individuals experiencing complex mental health and
psychosocial challenges to define their recovery and wellbeing goals, address and
reduce barriers to service access, and reduce avoidable hospital emergency
presentation.
The WTTCC program is
delivered within a holistic and integrated care model developed by Footprints
Community, in partnership with Brisbane North Primary Health Network (PHN),
Metro North Hospital and Health Service (HHS), and Micah Projects. This model delivers
wraparound, person-led care aimed at reducing repeated use of crisis services
and supporting sustainable changes and social inclusion.
About you
Essential
Qualifications:
* Bachelor's degree (or higher) in health services, social sciences, social work, or equivalent relevant experience in the human services sector.
Sector Experience:
* Extensive experience in the mental health sectors, with a sound understanding of service delivery for individuals with complex health and psychosocial needs.
Crisis & Risk Management:
* Proven ability to conduct comprehensive risk assessments, mitigate risks, apply de-escalation strategies, and follow the incident response procedures.
Recovery-Oriented Practice:
* Demonstrated ability to implement a recovery-focused and person-led approach that empowers participants to achieve goals and enhance wellbeing.
Relationship & Communication Skills:
* Strong interpersonal and communication skills, with the ability to build respectful, professional relationships with participants, colleagues, and external stakeholders while maintaining appropriate boundaries.
Teamwork & Collaboration:
* Ability to work both independently and as part of a multidisciplinary team in a dynamic, participant-focused environment.
Organisational & Technical Skills:
* Proficiency in using client databases and case management software, along with strong time management, task prioritisation, and reporting skills.
Compliance & Licences:
* Commitment to following organisational policies and procedures. A current Class C driver?s licence, and the ability to obtain (or possession of) an NDIS Worker Screening Check (Yellow Card) and Blue Card.
