The Transition Care Program is a short-term community-based service designed to support older Tasmanians after a hospital stay. The program delivers person-centred care, helping clients regain their independence and achieve their recovery goals.
The Nurse Manager will lead an interdisciplinary team providing compassionate, accountable, respectful, and excellent care. They will provide strategic and operational leadership, overseeing human, financial, and physical resources while ensuring compliance with relevant guidelines and standards.
Key Responsibilities:
* Lead and nurture an interdisciplinary team to deliver contemporary client-focused services
* Implement systems and processes to drive innovation and performance
* Manage resources, including information systems and data, to support the efficient operation of the service
Requirements:
* Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse with current AHPRA registration
Desirable Criteria:
* Demonstrated capabilities in leading and managing teams
* Proven leadership skills, including the implementation of systems and process
* Demonstrated capability to manage resources and make informed decisions
