Job description
Care Finder is a Commonwealth Government funded program commissioned by Primary Health Tasmania. The program aims to connect and engage older people who experience difficulty accessing aged care services. Care Finders provide one-on-one support for older Tasmanians to interact with My Aged Care and access aged care services and other supports in the community. Care Finders are based in each of Tasmania’s three major regions: South, North, and North West.
COTA Tasmania has been contracted as one of four providers of Care Finder services in Tasmania.

We are recruiting for a Care Finder/Intake Officer in our Hobart Office.
Care Finder:
* support people through registration, screening and assessment for aged care services;
* support people to identify, connect with and access services that are appropriate to their individual needs, preferences and circumstances;
* perform high level check-in with clients to see if services are still in place and meeting needs; and
* provide follow up support if needs change or services have lapsed.
Intake:
* be a warm and engaging person and the first point of contact for older people and referrers accessing the Care Finders program;
* quickly build rapport and trust with clients;
* determine their eligibility and circumstances;
* connect them with a Care Finder or facilitate referral onto other relevant support services; and
* maintain current information resources to support the Care Finders team.
Other key responsibilities include identifying opportunities for the delivery of the program, liaising with community organisations and networks to promote Care Finder services, and engaging with the target population through proactive outreach and rapport building. A strong commitment to connecting with and supporting vulnerable older individuals is essential, as is the ability to navigate and engage with diverse communities in Greater Hobart and regional areas including, Central Highlands, Southern Midlands, Glamorgan Spring Bay, Sorell, Tasman and Derwent and Huon Valley.
This is a fixed term contract until 30/06/2027, with the possibility of extension subject to funding.
The position is classified at Level 5.1 under the Social, Community, Home Care and Disability Services (SCHCADS) Award.
