About the role
We're looking for a passionate and experienced Senior Practice Nurse to join our multidisciplinary team and deliver high-quality, culturally safe, and holistic care to our community—including local Aboriginal and Torres Strait Islander people.
In this senior clinical role, you'll use your advanced nursing skills, autonomy, and leadership to support patients with chronic disease management, preventative health, and health promotion. You'll be a key part of enhancing access to care and driving better health outcomes across our region.
Key Responsibilities
Lead and support delivery of community health programs (e.g., smoking cessation, diabetes support, nutrition, early intervention).
Work collaboratively with GPs, Aboriginal Health Practitioners, Allied Health Professionals, and external partners.
Provide clinical leadership, mentoring and support to nursing staff and Aboriginal Health Practitioners.
Participate in case conferencing, quality improvement activities, and care planning meetings.
Empower patients through education and self-management guidance.
Maintain accurate clinical documentation using Medical Director.
Ensure compliance with AHPRA, clinical guidelines, and organisational policies.
Engage in ongoing professional development and clinical supervision.

About You
Current AHPRA Registration
Bachelor of Nursing (BN, BSc Nursing)
Understanding of Aboriginal health, cancer pathways, and community needs
Strong communication, teamwork and relationship-building skills
Australian citizenship or Permanent Residency
