**Our Journey**:
This is an exciting opportunity for an experienced midwife to work across all three maternity sites, promoting shared care as a viable care option for healthy well women with uncomplicated pregnancies. The role enhances support for women in shared care and optimises communication between our community shared care providers (GPs, midwives and obstetricians) and Monash Women's.
Should you be appointed, you'll be collaborating with external providers, working in a collegial environment and making a positive difference to the patient experience of pregnant women.
This is an ongoing position working 48 hours per fortnight.
**A Day in the Life**:
With varied responsibilities, in this role you can expect to:
Actively support women to enrol in the Shared Care Program (when identified by Maternity Bookings as being potentially appropriate);
Liaise closely with Maternity Bookings to ensure accurate registration of women referred in by a Shared Maternity Care Affiliate (SMCA);
Maintain an accurate database for women enrolled in Shared Care in order to monitor uptake and retention within the Shared Care Program;
Present a written summary report of activities to the GP & Shared Care Committee on a three to six monthly basis;
Provide a key point of contact for women, GPs, midwives and other hospital staff with non-urgent shared care enquiries; and
Support timely and accurate communication of investigation results (pathology, ultrasound) between Monash Women's and the community (and vice versa). This entails review of all Shared Care clinic patient lists, direct follow up with SMCAs in regard to missing results and the forwarding of results to Scanned Medical Record.
What You Bring:
- Significant experience in providing pregnancy care within the public hospital clinic setting
- Current knowledge of primary care services in particular general practice
- Ability to demonstrate computer and administrative skills including data base management
- Ability to work independently as well as within a collaborative team
- Excellent verbal, written and interpersonal communications skills
