Job Opportunity: Mental Health Professional
About the Role
* Conduct thorough client assessments and develop customised care plans to ensure optimal mental health support.
* Collaborate with healthcare professionals, including GPs, psychiatrists, psychologists, and allied health specialists, to deliver comprehensive patient care.
* Provide advocacy support for clients during medical and social appointments, prioritising their needs and well-being.
Key Responsibilities:
* Maintain accurate and timely documentation, and actively participate in team meetings to facilitate effective communication and collaboration.
* Contribute to mental health promotion initiatives, community engagement projects, and recovery-oriented care strategies to promote positive outcomes.
* Carry out home visits and community outreach programs to provide holistic support and enhance client experiences.
Requirements:
* Qualification in Social Work or a related field.
* 2-5 years of experience in community mental health or social support roles.
* Strong understanding of psychosocial support principles and trauma-informed care practices.
* Excellent communication and stakeholder engagement skills.
* Current WWVP check and valid driver's licence are essential.
