About The Role
We are looking for Youth Lead Practitioners to support adolescents and young adults aged 10–24 as they navigate life with a disability, from adolescence through to early adulthood. No two days will be the same. Responsibilities below illustrate typical activities you may undertake.
Responsibilities
- Lead the interdisciplinary team that plans a young person’s NDIS funded therapy package, acting as the main contact for adolescent and young adult participants and their families
- Utilise assessment and intervention skills to support youth participants to achieve their educational, personal, social, and working life goals
- Work within the social model across diverse settings (homes, schools, playgrounds, workplaces, community centres)
- Hold meaningful, future-focused conversations with participants and their supports
- Empower participants and their families to develop habits and routines
- Build meaningful relationships with participants and their supports (family, carers, friends)
- Document and bill for each session through the online participant management system
- Attend weekly team meetings in the Hub and share experiences and impact
- Meet with your Team Leader and Therapy Mentor to access clinical knowledge and support
Who We Are Looking For
We value collaboration and shared values. To work as a Youth Lead Practitioner, you will have a relevant tertiary qualification and registration.
- Youth Worker: Bachelor or Master’s degree in Youth Work or Community Services and YWA membership
- Social Worker: Bachelor or Master’s degree in Social Work and AASW membership
- Specialist Educator: Bachelor or Master’s degree in Education and AASE membership
- Registered Nurse: Bachelor of Nursing and AHPRA registration
You Will Also Need
- Minimum 12 months experience working with youth aged 10-24
- A positive and flexible approach to work
- Solid communication skills and be a team player
- Commitment to personal and professional growth
- Demonstrated experience working with people with a disability
- Willingness to work to billable targets
- A valid Australian driver’s license
- NDIS Workers Screening Check
- Working with Children’s Check
