As a Support Worker, you will provide person-centred active support to individuals living with disability. This role primarily involves Supported Independent Living (SIL), which includes assisting people with disability in either shared accommodation or their own homes.
* Support male and female clients in their daily living tasks.
* Assist with personal care tasks (e.g. bathing, dressing) and manual handling.
* Be available for short-notice shifts and sleepovers as they are essential.
* Implement person-centred active support and follow behaviour support plans.
* Support clients during mealtime routines to promote independence and safety.
* Log daily care and changes in needs to ensure informed coordinated support.
Key Responsibilities:
1. Personal Care Tasks: Assist clients with bathing, dressing, and other personal care activities.
2. Manual Handling: Provide safe and respectful assistance with lifting, transferring, and moving clients.
3. Mealtime Support: Assist clients with meal preparation, serving, and eating.
4. Coordination of Care: Log daily care and changes in needs to ensure informed coordinated support.
