General Medicine & Geriatrician Work Plan
Workdays / Hours :

Tuesday to Friday : *********
Daily Workflow :
********* : Huddle with ASET nurse
********* : Rounding on inpatients (allocated bed card) and inpatient consults
********* : ED consults (geriatric reviews, admissions, and patients for medical admission)
Key Responsibilities :
Early screening in ED using ISAR / frailty tools within 1 hour of presentation
Avoid unnecessary admissions (e.g. through HITH, subacute referrals)
Provide CGA in ED and wards
Review patients for RACF transfer under Gen Med / Geriatrics
Liaise early with discharge planner and ASET nurse
Support orthogeriatric shared care for older fracture patients
