Blue Mountains District ANZAC Memorial Hospital is an acute hospital that supports a population of approximately 80,000 and is the only ANZAC Memorial hospital remaining in NSW. Easily accessible to Sydney residents, our team of clinicians, allied health, and support staff works with the community on a shared goal to maintain the delivery of high-quality healthcare in the Blue Mountains region.

Located in South Bowenfels, the Lithgow Campus is considered as a comprehensive community health centre supporting approximately 20,000 people living in the town and surrounding areas. The services available in Lithgow hospital include Domiciliary care unit, Emergency department, Geriatric assessment unit, Obstetric services, Paediatric service, Drug and Alcohol unit

We are seeking a dedicated and compassionate Discharge Patient Flow Concierge to join our team working across both Blue Mountains District ANZAC Memorial Hospital in Katoomba and Lithgow Hospital. This role supports patients, families and carers as they navigate the final stage of their hospital journey, ensuring a smooth, coordinated and positive discharge experience. Working closely with the Administration & Patient Services Manager, you will play a vital role in strengthening our discharge planning processes and fostering seamless communication within the multidisciplinary team.

In this position, you will assist patients and their families throughout the discharge process, helping them transition safely and confidently back home or into other care settings. You will work collaboratively with clinical teams to streamline workflows, expedite timely discharges and enhance links with out of hospital care providers and primary health services. Your ability to communicate effectively, build strong relationships, anticipate needs and support meaningful conversations will be central to improving the overall discharge experience for patients, carers and families.

What You Will Bring To The Role

* Demonstrated extensive experience in customer service.
* Demonstrated high level administrative skills with ability to prioritise and multi-task.
* Excellent interpersonal, communication and negotiation skills, in particular the ability to interact effectively with a multidisciplinary team, patients, and carers.
* Demonstrated ability to work autonomously, utilise discretion and maintain strict confidentiality whilst working as effective team member.
* Adaptable to change with high attention to detail with ability to identify and implement change to facilitate system and process improvements.
* Demonstrated ability to problem solve and determine solutions.
* Demonstrated high level IT skills and knowledge of Microsoft Office software, including Excel and proficient keyboard skills.
