The Continuing Care Team at the Mackay Community Health Centre is seeking applications for a Mental Health Clinical Nurse to join their team on a permanent basis.

Your opportunity
* The successful post holder will function as a Mental Health Clinical Nurse as part of a multidisciplinary team responsible for coordinating and delivering case management and care coordination activities. The Clinical Nurse will manage a case load that supports individuals with serious mental illness to provide specialist assessment, crisis management, care coordination, delivery of nursing interventions, case review, advanced care planning and risk management to the Continuing Care Team.
* This role will also contribute to the planning, development, implementation, maintenance and evaluation of a broad range of treatment and prevention activities to promote recovery from mental illness.
About the Team
The Mental Health and AOD Service is comprised of eight clinical teams across the MHHS. The Mental Health Service is responsible for the provision of comprehensive, specialised mental health assessment and treatment services across the age-spectrum for people with severe and complex mental health problems. Our services aim to be consumer focused and recovery oriented and are provided in a variety of settings including community, an acute inpatient unit, hospitals and emergency departments.
