Opportunity
The NDIS Clinical Liaison Team seeks passionate and highly experienced Social Workers with strong expertise in disability and the NDIS to join our dynamic John Hunter Hospital-based team. This role plays a key part in delivering expert care coordination and discharge planning for patients living with disability, alongside their carers, and families.
Based at John Hunter Hospital, you will provide comprehensive, person‐centred social work interventions with patients and families, while offering specialist disability and NDIS consultation to multidisciplinary colleagues across acute care settings.
This position is ideally suited to a motivated and confident Social Worker who thrives in fast‐paced hospital environments and enjoys sharing specialist knowledge to support best practice care.
Successful applicants will receive orientation to the NDIS Clinical Liaison Team, and will have strong professional connection, supervision, and close collaboration with the John Hunter Hospital Social Work Service.
Responsibilities
* Deliver expert care coordination and discharge planning for patients with disability, interfacing with the NDIS and community services.
* Provide person‐centred social work interventions and specialist disability and NDIS consultation to multidisciplinary teams.
* Support and supervise social work staff and/or students, in accordance with professional and organisational supervision frameworks.
Qualifications
* Bachelor in Social Work or qualifying Masters; eligible for membership of the Australian Association of Social Workers (AASW).
* Eligibility to drive in NSW.
* Advanced knowledge of challenges experienced by people with disability, including understanding of the NDIS, discharge planning, and transition to community settings.
* Sound understanding of NSW Health policies, frameworks, and strategic plans relevant to disability and NDIS interface arrangements.
