Healthiest Version of You provides compassionate, tailored dietetic care to help NDIS participants and those with complex health needs achieve their nutrition, health, and wellness goals. Our approach is friendly, non-judgmental, and empowering, supporting independence in areas like nutritional management, meal planning, building healthier relationships with food and making better food choices. We offer services including individual consultations, meal planning, report writing, nutrition management plans, grocery shopping guidance, and staff training to support workers and family. With expertise in NDIS processes, we advocate for participants and collaborate with care teams to ensure access to needed nutrition support.
Role Description
This is a part time on-site role for a Dietitian located in Gold Coast or Logan region. The position starts at 2 days per week with room to grow as the caseload increases. It's a mobile role, so the successful applicant will need a driver's license and reliable car to travel around.
Key Responsibilities
* Conduct nutrition and dietetic assessments and provide nutrition interventions for NDIS participants across all age groups, mostly 2 years and up.
* Train support workers and family members on the participants' nutrition needs.
* Develop high-quality NDIS reports and record thorough clinical case notes.
* Provide face-to-face support in home/community settings and telehealth appointments when appropriate. Note we don't have any clinics as we are solely mobile.
* Collaborate with stakeholders to ensure integrated participant care.
Qualifications
* Own car and driver's license
* Membership with Dietitians Australia
* Working with Children Check
* CPR and First Aid certified
* 1+ years' experience in private practice or hospital
* Professional Indemnity & Public Liability insurance (min. $10 million)
* Police Check
* NDIS worker screening card
* Ability to work autonomously on a day-to-day basis, as well as collaboratively with other health professionals of NDIS participants
* Experience with NDIS participants
