About The Team
The Building Strong Foundations (BSF) program is part of the Children, Young People and Families Service and is based at Taree Community Health Centre on Biripi Land. The team delivers culturally safe, compassionate and evidence-based child and family health nursing services to Aboriginal children aged 0–5 years and their families across the Greater Taree region. Working as part of a multidisciplinary Child Health Team, you will partner closely with the BSF Aboriginal Health Worker, Aboriginal Maternal and Infant Health Service (AMIHS) and local Aboriginal communities to provide integrated, person-centred care that supports positive health and developmental outcomes for children and families.
About The Role
Role: Child & Family Health Nurse / Clinical Nurse Specialist Grade 2
You will:
* Provide culturally sensitive, community-based nursing care to Aboriginal children and families.
* Support infant and child physical, emotional and social health using evidence-based practice.
* Manage a diverse and rewarding caseload, often supporting families experiencing psychosocial complexities.
* Deliver care in clients' homes, community settings and the Community Health Centre.
* Work collaboratively with Aboriginal communities and multidisciplinary health teams to improve health outcomes.
* Provide assertive follow-up and ongoing support to Aboriginal children aged 0–5 years and their families/carers.
This role is a job-share opportunity working alongside another CNS-2 within the BSF team.
