Community Worker – Job Duties

* Deliver person-centred assistance and connect clients with relevant relief and community support services.
* Conduct client assessments and implement tailored action plans to address social and economic needs.
* Identify service gaps within the community and liaise with agencies to improve access to essential supports.
* Represent and support disadvantaged groups to strengthen inclusion and community participation.
* Facilitate skill-building sessions to enhance self-reliance and everyday living capabilities.
* Provide immediate support and appropriate referrals during crisis or high-risk situations.
* Organise and oversee outreach activities, including community-based support and relief programs.
* Support clients to attend appointments and participate in social and wellbeing activities.
* Maintain accurate case notes, reports and documentation in line with organisational and compliance requirements.
* Collaborate with multidisciplinary teams and external stakeholders to coordinate comprehensive client support.
* Monitor client progress and review support strategies to ensure outcomes are effectively achieved.

Job Type: Full-time

Pay: $37.00 – $38.00 per hour

Expected hours: 36 per week

Work Location: In person
