You'll conduct initial psychosocial assessments, collaborate with our multi-disciplinary team, and help guide each young person towards care and support they need. - Monitor and conduct psychosocial assessments on headspace Emerald young people to ascertain social, emotional, and mental health needs. - Deliver evidence based brief interventions, including group interventions and problem solving with young people and their families. - Provide an inclusive practice that values the significance of family and friends. - Attend and contribute to regular Care Coordination Review Meetings. - Assist and provide appropriate and timely referrals for a range of treatments and services offered by headspace Emerald's partners, Private Practitioners, and other key agencies. Mandatory Requirements: - Diploma in Social Work, Mental Health, Psychology, Occupational Therapy or relevant.
