We are seeking acompassionate, motivated, and skilled SocialWorker/(s) to join our team. The successful applicant/(s)shall provide social work services to patients. The Social Workershall be responsible for assessing clients' needs, providingemotional and practical support, developing care plans, andconnecting individuals and families to appropriate resources andservices. This role involves advocacy, case management, crisisintervention, and ongoing client support to promote well-being,self-sufficiency, and community integration. Facilitates andpromotes patient safety and quality of care. Practices as a SocialWorker as per the Australian Association of Social WorkersPractices Standards 2013 and Code of Ethics 2010 and/or the Societyof Professional Social Workers Code of Ethics and SMHS policies andguidelines.
