We're seeking a Clinical Team Leader (Registered Nurse) to join our dedicated team at Just Better Care Mid North Coast. This is a unique opportunity to combine clinical expertise with team leadership, ensuring the delivery of safe, high-quality and person-centered care across our community.
The role
As a Clinical Team Leader, you will:
* Lead, supervise and support a team of nurses and Community Support Professionals fostering a collaborative team environment.
* Oversee the development, implementation and review of individual care and health plans, ensuring alignment with best practice and participant goals
* Conduct comprehensive clinical assessments and respond proactively to changing health needs and risks.
* Coach, mentor and train staff in clinical skills and competencies, fostering a culture of continuous improvement.
* Collaborate with participants, families, and health professionals to deliver integrated, respectful, and outcome-focused services.
* Monitor risks, respond to changing participant needs, and ensure compliance with aged care, disability, and health standards
* Contribute to organisational performance through incident management, quality improvement initiatives, and strategic service delivery.
Duties and responsibilities may include:
* Working with or under the guidance of medical practitioners and Allied Health professionals
* Medication Administration and Complex Clinical Care eg. palliative care, bowel care/suppositories, catheter care, stoma care, oxygen and more
* Monitoring and communicating on the customer's wellbeing and reporting on progress of customers
* Ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care's policy and procedures, legislative requirements and program specific guidelines (i.e. DVA Community Nursing)
* Providing training, coaching and guidance to Community Support Professionals, Enrolled Nurses and Support Staff/Case Managers as required
* Conducting skills assessments of Community Support Professionals performing complex care services as required
* Demonstrating a commitment to best practice and evidence of continued professional development
* Participating in staff meetings and attending training opportunities when required
What you will need:
* Current Registered Nurse registration with Australian Health Practitioner Regulation Agency (AHPRA)
* Evidence of continued professional development in accordance with the AHPRA standards
* Current Criminal Record Check and Working with Children Check
* Current CPR / First Aid Certificate
* Current Australian Driver's Licence
* Excellent leadership, written and verbal communication skills
* Computer literacy
* Commitment to person-centered, trauma-aware and culturally safe care
* Experience in the supervision of staff members
* Knowledge of continuous quality improvement principles
* Excellent people, problem solving, and conflict management skills
* Ability to work as part of a team and maintain good working relationships
Desirable:
* Previous experience in the community care or health/aged care sector
* Understanding of DVA Community Nursing guidelines or previous community nursing experience
* Cert IV in Workplace Training & Assessment
