Your Opportunity
As a Case Manager at Hambledon House Community Centre in Edmonton, Cairns, you will deliver client-centred, comprehensive case management services that make a meaningful difference for children, families, and individuals across the Cairns South community. Working under general supervision, you will apply initiative, sound judgement, and specialist knowledge in complex situations where established procedures may not always be clearly defined, while managing an active caseload of approximately 15 families. You will collaborate closely with local services and community groups, contribute to outreach family support services, and support high-quality outcomes for families accessing universal and targeted supports, including the Food Pantry program. The role may include guiding and supporting junior staff, creating opportunities to build leadership capability within a values‑driven, community‑focused environment.
Your key responsibilities will be to:
* Provide practical, family-centred case management support, including assessment, intervention, advocacy, and referrals.
* Manage a caseload of up to 15 families with complex needs, supporting goal setting and progress review.
* Support families to access education, employment, health, and community services.
* Develop strengths-based support plans in partnership with families.
* Maintain accurate case notes and records in line with privacy and program requirements.
* Build positive relationships with families, community partners, and service providers.
Requirements for Success
* Certificate IV or higher in Case Management, Community Services, Youth Work, or a related field.
* Substantial experience delivering complex, strengths-based case management and support to families, children, and young people.
* Experience working in outreach and community-based settings, supporting families to build capacity and independence.
* Proven ability to build effective relationships with families, community members, and external service providers.
* Strong communication, organisational, and record-keeping skills, with the ability to manage competing priorities.
* Ability to work autonomously as a self-starter, using initiative and sound judgement.
* Previous experience in family support, community services, or a related case management role.
* Hold a Current QLD full Driver's Licence.
