Job Summary
We are seeking a highly skilled Nursing Professional to lead our Preventative Integrated Care Service. This role requires expertise in contemporary Respiratory knowledge and skills, with a focus on guiding high-quality, safe clinical services across the health system.
Main Responsibilities
* Provide clinical nursing leadership within the Preventative Integrated Care Service, working collaboratively with Nurse Practitioners and Medical Officers to ensure safe delivery of nursing care to patients with chronic conditions.
* Develop and implement evidence-based clinical practice guidelines to inform service delivery.
* Coordinate nursing and provide health services across the continuum of care, ensuring seamless transitions between acute and community settings.
* Lead the development of digital solutions to enhance patient care and outcomes.
Required Skills and Qualifications
* Masters degree in Nursing or related field.
* Registration or membership with the Australian Health Practitioner Regulation Agency (AHPRA).
* Advanced interpersonal, communication, and time management skills.
* Proven experience in clinical leadership and service development.
