Help clients live independently and safely while building meaningful routines. Provide one-on-one psychosocial support and deliver trauma-informed, person-centred support in home and community settings Support emotional regulation, daily living activities, and community access Build trust and maintain professional, therapeutic boundaries Assist the client with transport to appointments, social activities, and community programs Build trust and maintain professional, therapeutic boundaries Implement positive behaviour support and de-escalation strategies when required About You Minimum 6 months' experience or Cert III/IV in Disability, Aged Care, or other related fields is highly desirable Experience working in in-home care / community setting Psychosocial support, personal care experience are highly desirable Strong understanding of professional boundaries, restrictive practices, and behaviour support Effective written and verbal communication, including clear case note writing Able to set priorities, meet deadlines, and work under pressure with minimal supervision Strong interpersonal skills to work both autonomously and as part of a team NDIS Worker Screening, First Aid, Driver’s Licence, and insured vehicle If you’re passionate about making a real difference and ready to build a rewarding career, apply now and help us empower individuals to live with freedom and choice.
