Our model of care provides holistic health and wellbeing services to support our Community across their lifespan. We bring together primary care, allied health, dental, mental health and wellbeing, maternity and early childhood, disability services, specialist services, and aged care as one team focussed on clients' individual needs and goals.We are an integrated team working together to improve the health and wellbeing of our Community. Our network is growing across Caboolture, Morayfield, Strathpine, Deception Bay, and Margate – now there's an opportunity for you to be part of a great team making a real difference in people's lives.**About the role**Staying Deadly Care Coordinators provide outcome focused health, social and practical support to Aboriginal and Torres Strait Islander clients. Working within a multidisciplinary team to support clients with moderate to severe mental health needs that; are at risk of entering the acute care system if their symptoms are unmanaged, or who are stepping down from an inpatient episode of care to ensure they are well supported in the community through holistic and wrap around primary health and mental health care. The role will manage a caseload of clients with varying needs, to empower and foster strength, resilience and well-being through participation in intensive case management, planning and coordination.Your day to day duties may include:* Developing and maintaining support plans in conjunction with clients and relevant service providers.* Work with clients in a trauma informed, recovery and/or strengths-based approach to formulate, implement and review well-being plans including a comprehensive assessment of need and safety planning.* Assisting in the coordination and delivery of services for clients identified with drug and alcohol, mental health, chronic disease and social health issues.* Conduct home, community and/ or custody visits to support ongoing client recovery.* Liaising and developing external relationships with services/agencies to address client needs.* Developing networks and building community support for promotion and participation in services**About you****To perform this role, it is essential that the incumbent is an Aboriginal or Torres Strait Islander person.
