Job Title: Community Navigator
We are seeking a permanent, full-time professional to connect people with housing and related resources.
The ideal candidate will have strong written and verbal communication skills, as well as up-to-date knowledge of housing information, community resources, and service providers.
This role requires a commitment to collaboration with our partners and a willingness to develop relationships across various sectors.
* Conduct assessments to determine the best course of action for each individual;
* Provide information and advice on available services and support options;
* Assist with social housing, private rental, and other housing assistance applications;
* Collect and record relevant data and information;
* Facilitate access to specialist services and mainstream opportunities;
* Source local networks, resources, and opportunities that can benefit service users;
Key Responsibilities:
* Initial and comprehensive engagement and assessment processes;
* Light touch (Level 1) or short to medium term intervention (Level 2);
* Coach and support service users to develop and progress an initial response or action plan;
* Feedback on progress with action plans;
