About the Roles:
SUPPORT WORKER ROLES:
We are seeking Disability Support Workers to support individuals with a disability to live a meaningful and fulfilling life and to help deliver accessible, compassionate, reliable and high-quality services to participants. Your duties with Crafted Independent Living will vary greatly, depending on the person you are supporting, and will include lifestyle support, daily living support, community access, healthcare support etc.
Common Support Worker Roles and Responsibilities:
LIFESTYLE SUPPORT
Provide support to enable the person to live their chosen lifestyle.
Facilitate contact with family, friends and associates.
Provide support to assist the person in making informed decisions.
Advocate for and with the consumer, where appropriate.
Supporting participants to maintain and improve their physical, emotional, financial, social, and cultural well-being.
Support person-centred plans that identify a participant's life goals and help the participants to work toward achieving their desired outcomes.
DAILY LIVING SUPPORT
Assist with self-care, general hygiene, and personal presentation.
Assist with cooking, washing, ironing, and house cleaning.
Assist with shopping for food, clothing, and personal items.
Encourage independence in daily living activities, where appropriate.
COMMUNITY ACCESS SUPPORT
Assist the person to become an active member of their local community.
Assist with transport and accompany the person on outings and activities (may include support to use public transport).
HEALTH CARE SUPPORT
Arrange medical appointments and collect prescriptions.
Oversee, administer medication as appropriate.
Transport the person to and from appointments and attend as required.
Monitor the person's general health and well-being.
FINANCIAL MANAGEMENT SUPPORT
Assist the person to prepare and develop a weekly budget.
Assist the person with banking, paying bills and keeping receipts.
GENERAL
Be aware and take personal responsibility for the safety of yourself and the people you support, reporting all hazards and incidents to management.
Identify risks and hazards in the workplace and minimise or remove in a timely manner.
Be punctual, accountable, proactive, and efficient across all environments.
Be a good role model and promote positive behaviours.
Utilise sound organisational and planning skills.
Work autonomously and seek support when required.
Always act in a way that is in the best interest of the participant and their families.
Respect the participant's dignity, privacy, and confidentiality.
Contribute to the participant support arrangements using initiative, sensibility, innovation, and creativity.
Complete daily recordings and individual goal outcomes
You are friendly, have a can-do attitude and can work as part of a team.
ESSENTIAL CRITERIA
Excellent communication and listening skills.
High level of initiative & great problem solving skills.
Have demonstrated an ability to view people with disability as valued individuals with human rights.
Are committed to a person-centred approach.
Certificate in disability work, aged care or desire to obtain.
Current Yellow Card / NDIS Worker Clearance (or be willing to obtain)
Current First Aid Certificate with CPR (or be willing to obtain)
National Police check obtained within the last 12 months.
Valid Queensland Driver licence and access to registered, insured vehicle.
Able to work in Australia with no restrictions on hours.
Must be fully vaccinated against COVID-19 or be willing to be vaccinated ASAP.
DESIRABLE CRITERIA
Some positions require you to be available to work a 24/7 roster which includes evenings, sleepovers and weekend shifts.

**Salary**: $30.00 - $38.08 per hour
