This may include:provision of education/advice on your condition and recommended management strategiesprescription of home exercise programindividual recommendations and setup of aids for walkingreferral to other health care professionals or community exercise groups.Our service places a strong focus on health promotion and active self-management.Working in collaboration with other health professionals, Physiotherapists are also actively involved in chronic disease prevention, physical activity and health promotion activities in various healthcare and community settings.The Role:As part of the Community Physiotherapy team, and in accordance with organisational policies andprofessional code of ethics, be responsible for the efficient and effective provision of physiotherapy servicesto clients living in the community in the region.Work as part of a multidisciplinary team in providing appropriate services to clients and patients at primaryhealth sites, including referral to other services as required.Participate in ongoing development and coordination of the services provided by CommunityPhysiotherapists in the Northern area, including supervision and support of staff and students where required.Participate in the development and monitoring of policies and procedures, performance measures andreporting mechanisms for physiotherapy services provided by the Community Physiotherapy Service.Contribute to health promotion within the region.Maintain contemporary professional knowledge through applying evidence based practice and appropriate continuing professional development activities.
