We have an exciting opportunity for a Senior Planned Care Nurse to join our Primary Care Team at HMP Altcourse, a Category B prison located in the Fazakerley area of Liverpool. This very busy remand prison has a high turnover of patients, and this role will be an integral part of the healthcare team delivering a nurse‐led service and preventing hospital admissions.
Hours
We operate a 7‐day a week service. You will be expected to work approximately 1 in 3 weekends and some nights. Shift times include 7 am–8 pm, 8 am–9 pm, 9 am–10 pm and 7:45 pm–7:15 am.
Salary
This full‐time position offers an annual salary from £43,609 to £49,175 per annum, depending on experience.

Responsibilities
* Provide visible, effective leadership and act as a clinical role model.
* Support staff with complex clinical decisions and promote a culture of safety, learning, and continuous improvement.
* Carry out planned care management and lead and coordinate programmes such as vaccination, screening and health promotion.
* Oversee management of long‐term conditions (diabetes, asthma, hypertension, COPD).
* Line‐manage and support the Practice Nurse in delivering preventative and planned care.
* Monitor and audit planned care pathways to ensure clinical governance compliance.
* Rota and workforce management: plan rota and staff allocation to maintain safe staffing levels.

Qualifications required
* Registered nurse (Adult).
* Current professional qualification (NMC).
* Evidence of continued professional development.
* Understanding of the complexities of multi‐disciplinary and multi‐agency working.
* Excellent communication skills.
* Proactive problem‐solving skills.
* Motivated as an individual and when working in a team.
Although not essential, ideal applicants will have experience in a hospital, community, or prison environment.
