About the Role
As a Care Partner, you will be responsible for providing personalised care journeys for our valued clients. This involves being the primary advocate and contact for clients across various programs.
You will work closely with our Clinical Team and Allied Health providers to deliver exceptional wellness and reablement outcomes. Your key responsibilities will include designing and overseeing comprehensive care plans aligned with the Aged Care Act.

Requirements
To be successful in this role, you will need to have a proven industry background in aged care, preferably In-Home Services, with a solid coordination experience. You will also need to possess a Certificate IV in Aged Care/Community Services or a relevant tertiary qualification.
Additionally, you should have a demonstrated commitment to enhancing the lives of older Australians, a reliable work ethic, attention to detail, current National Police Clearance, and Australian citizenship or valid visa with working rights.
