About the Role
The primary role of the Outreach Support Worker is to provide general case management support for clients, to empower them to make positive changes in their lives. The Outreach Support Worker will work closely with other members of the MHW team to ensure the appropriate level of support is provided by the team, including by other MOSAC teams or staff if necessary. The Outreach Support worker may take primary responsibility for delivering one or more of the programs offered by the MHW team, with support from team members and the Team Leader.

Key Responsibilities
· Provide case management support to clients making the transition from prison to community
· Support the development and implementation of culturally appropriate programs for clients, family members and community groups
· Enter client data into a client management system and create reports
· Support the development of culturally appropriate programs.
· Make referrals for clients to specialist health services.
· Attend and participate in team de-briefs, staff meetings and training as required
· Contribute towards ensuring a safe workplace for all staff, by following policy and procedure; and report any issues of concern to the Team Leader MHW.

What We're Looking For
· Previous experience working with Aboriginal men and families
· Current Western Australian "C" class driver's license (Manual)
· Ability to undertake all training required for MOSAC staff
· Ability to obtain National Police Certificate and prison clearance

Desirable
· Knowledge and work experience in complex issues such as mental health, substance abuse and family and domestic violence
· Ability to communicate with men from diverse backgrounds.
· Experience in facilitating workshops or confidence to learn.
· Good time management, written and verbal communication skills and planning and organisation skills.
· First Aid Certificate
