Job Description:
As a Clinical Care Coordinator, you will play a vital role in ensuring the delivery of safe and effective care to residents in our 77 bed Aged Care Centre. Located in New Town, TAS, Australia, our centre provides a cosy and comfortable home with mountain and park views.
The successful candidate will have a minimum four years of clinical experience as a Registered Nurse, preferably in aged care. You will be responsible for coordinating the delivery of care services, promoting a person-centred culture, and ensuring that residents receive individualised care plans.
Key responsibilities include:
* To provide clinical leadership in the coordination of nursing and care services
* To promote and support a person centred culture and values - respect and treat all people as unique individuals with needs, preferences and a past history
* AN ACC understanding in relation resident care plans, and ensure that sufficient evidence is maintained for validation of all claims
* To maintain accountability and responsibility for the clinical management of health and care outcomes of the residents in the Centre
* To ensure that skills and knowledge appropriate to delegate the task/authority has been considered when delegating health and care responsibilities to other professionals
* To exercise critical thinking within an evidenced based framework to ensure the delivery of high quality health and care services which promote optimal health outcomes for the residents
* To deputise in the Care Manager's absence
Requirements:
To be successful in this role, you will need to possess excellent organisational and time management skills, high level verbal and written communication skills, and a strong understanding of aged care legislation and AN-ACC standards.
You must also be a self-starter and able to work autonomously, with a strong commitment to continuous quality improvement and a passion for delivering high quality care to residents.
