Care Partner

We are seeking an experienced and compassionate care partner to join our Care Services Team and support older people to live independently and with dignity in their own homes.

About the Role

As a care partner, you will:

• Provide person-centred case coordination to older clients receiving aged care services

• Conduct comprehensive needs assessments and develop individualised care plans

• Work collaboratively with clients, families, and service providers to achieve client goals

• Support clients to navigate aged care systems

• Monitor service delivery, review outcomes, and adjust supports as required

• Maintain accurate documentation and compliance in line with the Aged Care Quality Standards

• Advocate for clients and ensure culturally safe, trauma-informed practice

You will work closely with a multidisciplinary team committed to delivering high-quality, culturally responsive services.

Essential Qualifications & Experience

• Relevant tertiary qualification in social work, community services, nursing, allied health, or related field

• Demonstrated experience (minimum 2 years preferred) in aged care, home care, case management, or care coordination

• Strong understanding of the Australian aged care system

• Experience supporting older people from multicultural backgrounds

• Demonstrated ability to develop care plans, conduct assessments, and manage client caseloads

• High-level interpersonal, communication, and documentation skills

• Strong organisational and time management skills

• Ability to work both independently and collaboratively
