About the Role
We are seeking a highly skilled Care Coordinator to join our team in Newcastle and the Hunter Region. As a key member of our Aged Care services, you will be responsible for overseeing and enhancing the delivery of our care services.
The successful candidate will have a strong understanding of Aged Care funding and service delivery, as well as experience in clinical case management. They will also possess excellent communication skills and be able to effectively build relationships with diverse stakeholders.
In this role, you will be responsible for processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit evolving needs. You will also coordinate home visits to ensure the needs of our clients are met and that we are working in partnership with them and their families.
Key Responsibilities
* Client Assessments: Undertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholders.
* Care Plan Monitoring: Ensure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visits.
* Risk Management: Monitor the clinical risks and needs of a client, ensuring referral pathways are established for the safety of our clients.
* Collaboration: Collaborate effectively with internal stakeholders to ensure clients' needs are met including our Rostering team and Practice Leads.
* Aboriginal and Torres Strait Islander Services: Develop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities.
Required Skills and Qualifications
* Aged Care Experience: Demonstrated Aged Care or other clinical case management experience.
* Technology Skills: Confidence in utilising a variety of technology including mobile devices and computers.
* Software Proficiency: Proficiency with Microsoft Office, Outlook and database systems.
* Communication Skills: Strong communication skills and confidence in delivering training.
* Time Management: Ability to multi-task and work autonomously in a fast-paced environment.
* Transportation: A current unrestricted Australian Drivers Licence, reliable vehicle and a willingness to travel.
