Overview
Acute to Aged Related Services - Clinical Nurse Specialist 2 role at Hunter New England Local Health District. Location: Armidale Hospital. Employment Type: Permanent Part Time. Hours per week: 30. Remuneration: $57.11 - $58.99 Per Hour + Super + Salary Packaging.
Responsibilities
- Provide advanced clinical care, leadership, and expertise in managing complex cases within the Acute to Aged Related Services (AARCS) team.
- Deliver the first face-to-face assessment within one working day of referral and work with families, carers, and service providers to develop safe discharge plans.
- Balance competing priorities, manage patients exhibiting challenging behaviours, and advocate for older patients while guiding and supporting colleagues.
- Apply advanced assessment and case management skills to contribute to improving outcomes for older people.

- Participate in interdisciplinary collaboration to ensure patient-centred care and timely discharges.
Qualifications
- Current registration as a Registered Nurse with AHPRA.
- Relevant post-registration qualifications and experience appropriate to the role of CNS2.
