Momentum Community Connections exists to encourage participation, promote inclusiveness, and create community connections by providing a wide range of support services and programs to Victorians aged between the ages of 17 and 65 living with disability to lead an independent and happy life.
ABOUT THE POSITION
We're seeking a reliable and committed Support Worker who shares a strong passion for AFL football to support one of our participants who absolutely loves the game.
This role is ideal for someone who genuinely enjoys AFL, is happy to chat all things footy, attend games (both local and AFL-level), and even kick a few goals if needed Your shared interest will help build a strong connection and make support feel less like work and more like mateship.

You MUST have the following:
* A genuine love for AFL – must be happy to attend games and have a kick.
* Someone comfortable driving to different AFL games and locations in Melbourne, including local and professional matches
* A high level of reliability and commitment to regular, ongoing shifts – consistency is incredibly important to the participant. You must be available to workTuesday's and Wednesday's between 9:30 am and 7 pm. Mondays, Thursday and Friday's from 4:30 pm onwards until 11 pm.
* Experience working with NDIS participants and implementing strategies from BSP's to work with behaviours of concern/s.
* Must be able to travel to Bacchus Marsh as the shifts start and finish from there.
KEY RESPONSIBILITES
* Empower participants by assisting with community access shifts, encouraging social inclusion and helping them engage confidently in local activities and services.
* Work closely with NDIS participants to implement tailored strategies from their BSPs, ensuring a safe, supportive environment that fosters positive behavioural outcomes.
* Work as part of a team of professionals to deliver holistic support, ensuring that all participants receive comprehensive, person-centred care.
* Documenting & progress note keeping.
MANDATORY REQUIREMENTS
* NDIS Screening Check (willing to obtain one).
* NDIS Orientation Modules
* WWCC- Employee.
* Drivers Licence and car insurance.
* Ability to commit to a rotating roster including weekends and public holidays.
* Smart Phone.
