RACF Outreach Nurse Practitioner/Transitional Nurse Practitioner
As a specialist healthcare professional, you will be responsible for delivering high-quality care to residents in Residential Aged Care facilities within the district. You will collaborate with a multidisciplinary team to ensure comprehensive specialist care is provided at the right time and in the right place.
Clinical Responsibilities:
* Provide evidence-based extended and advanced nurse practitioner care for patients registered with the SLHD RACF Outreach Service.
* Facilitate and support the transition of inpatients to residential aged care prioritizing those experiencing severe behavioural and psychological symptoms of dementia.
* Assist with the management of complex comorbid patients expected to transition to residential aged care in the near future.
Leadership Responsibilities:
* Provide clinical leadership and supervision to nursing staff within the SLHD RACF Outreach Service.
* Develop and enhance the scope of practice for Transitional Nurse Practitioners and Nurse Practitioners working within the SLHD RACF Outreach Service.
* Work in conjunction with the Nurse Unit Manager and the Medical Director to ensure all nursing staff are supported to work at their full scope of practice and access professional development opportunities.
