The Access Aged Care team provides co-managed primary care services to Aged Care residents across Australia.
Your New Role
As you step into this exciting onsite part-time opportunity reporting to the Nursing Regional Manager, you become an integral part of our organization in a newly created position dedicated to improving health outcomes for our valued customers.
This will be achieved through providing clinical management and coordination for our clients and partnering residential aged care facilities. The role is for a motivated and passionate individual capable of working autonomously and appreciating the flexibility of this role, which will be based at participating Aged Care facilities. Regular travel to partnering homes for clinical reviews will be required. The position is advertised for Huonville, but travel to other areas may also be necessary.
The Enrolled Nurse will work as part of a multidisciplinary team to ensure efficient, high-quality service delivery, coordinating the clinical team with support from the Nursing Regional Manager and administration team. The successful applicant will also support business development to grow the resident base. Proficiency in computer usage is essential, and prior telehealth experience is preferred but not mandatory.

Essential Criteria
* Diploma of Nursing
* Experience in problem-solving and professional collaboration
* IT skills and proficiency in documenting customer needs
* AHPRA Nursing Registration
* Aged Care experience, leadership preferred
* Current driver's license and reliable vehicle
Skills & Experience
* Experience in similar roles ensuring individual health needs are met
* Organization and time management skills
* Relationship-building and excellent communication skills
* Proficiency in computer usage
* Aged Care experience and understanding of clinical scope
* Knowledge of person-centered care
